Time Warner Cable Cup charity donation

The Time Warner Cable Cup follows the rivalry between the Albany River Rats and the Adirondack Phantoms. The two American Hockey League teams play each other 12 times in the upcoming 2009-2010 season. The team that wins more Cup games wins the Time Warner Cable Cup and the opportunity to help designate a local non-profit to receive $5,000 from Time Warner Cable.

This is your chance to nominate your organization to be considered for the Time Warner Cable Cup $5,000 award.

Complete the nomination form below. All submissions are due no later than Feb. 1, 2010. Simply fill in the required fields and describe how your mission and/or program answers the following question (please be as specific as you can):
