There is a $25.00 processing fee for approved requests, which must be paid in advance and can be remitted by mail to the above address with a check or money order payable to: "Time Warner Cable."

If you wish to use your credit card then please submit the below form and a representative will contact your for your credit card information.

If you are a Time Warner Cable subscriber in the Central New York or Southern Tier divisions you have the option of having this fee added to your bill**.

Please allow 4-6 weeks for delivery.

This form may not be used for any requests for video to be utilized for legal, investigative, or commercial purposes. Those requests must be made by letter to the address above. Copies are for personal viewing use only.

I understand, and agree, that the copy provided by Time Warner Cable sports may not be duplicated, exhibited or licensed for any other purpose, and I agree that Time Warner Cable shall not be liable to me in connection with my requst for the copy or the content thereof.
