The Reward Card plastic is valid through the expiration date shown on the front of the Reward Card or until the value on the Reward Card reaches zero. The funds on the Reward Card do not expire. If there is a balance remaining after the expiration date shown on the front of the Reward Card, you can call 1-800-571-1376 to request that a replacement Reward Card be issued to you. The replacement Reward Card will have a value equal to the remaining balance of the expired Reward Card minus a Re-Issuance Fee. A service fee of $2.50 per month beginning the 7th month from the date of activation will be charged to any remaining balance except where prohibited by law. Your card may have been activated up to two months prior to receipt. Please refer to Cardholder Agreement for more information.
Card holder should call 1-800-571-1376 to report a Visa Reward Card lost or stolen. The card holder is required to provide their name, the card number, original value and transaction history. If records show a balance still remains on the card, the original card will be canceled and a refund of the balance amount will be issued in the form of a new standard design Visa Reward Card. There is a $5.95 Re-Issuance Fee for replacing a Visa Reward Card.
